From Evidence to Enduring Impact: Building Adaptive Primary Care and Population Health Systems

Health systems across the Asia Pacific face growing pressures from population ageing, chronic disease, health inequities, and rapid social and technological change. Drawing on Singapore’s experience in population health and integrated care transformation, this plenary examines how primary, community, and acute care can be connected around people’s needs. It explores the roles of data, implementation science, and community partnerships in translating evidence into practice, and argues that resilient health systems must continuously learn, adapt, and improve to achieve better health, longevity, and equity.
